what is ApiFix surgery?
A scoliosis diagnosis can bring a wave of emotions — concern, confusion and uncertainty. It’s understandable to have questions and to want the best, least disruptive treatment possible for your child. Fortunately, there are more treatment options today than ever before, including ones that are less invasive and designed specially for growing kids.
At Dayton Children’s Hospital, we offer ApiFix®, a surgical option for eligible children and teens that helps straighten the spine with smaller incisions, a quicker recovery and excellent long-term results. The spine team at Dayton Children’s has performed more ApiFix procedures than nearly anywhere in the United States.
ApiFix is an FDA-approved surgical treatment for moderate scoliosis in adolescents. It works by gently straightening the spine using a flexible rod and a self-adjusting design. Unlike traditional spinal fusion, ApiFix allows for some movement and flexibility even after surgery.
This approach uses just a few small incisions and avoids the need to fuse large sections of the spine, making it a less invasive option that preserves spinal motion.
who is a candidate for ApiFix?
ApiFix is an exciting treatment option, but it’s not right for everyone. It’s designed specifically for children and teens who meet certain criteria. Your child may be a good candidate for ApiFix if they:
- Have adolescent idiopathic scoliosis (AIS): This is the most common type of scoliosis, typically diagnosed during growth spurts in late childhood or early adolescence.
- Are still growing: ApiFix works best in patients who haven’t finished growing yet. Ongoing growth helps the spine continue to correct and adjust after surgery.
- Have a spinal curve between 35 and 60 degrees: This range is ideal for ApiFix’s corrective capabilities. Curves below 35 degrees may not need surgery, while curves above 60 degrees may require a more traditional approach.
- Have flexible curves that can be corrected without spinal fusion: ApiFix relies on the spine’s natural flexibility to gradually guide it into alignment. Flexibility is typically assessed through bending X-rays or other imaging studies.

FAQs – ApiFix
The ApiFix system is indicated for use in patients with adolescent idiopathic scoliosis meeting the following criteria:
- Single curves classified as Lenke 1 (thoracic major curve) or Lenke 5 (thoracolumbar/lumbar major curve)
- Cobb angle 35 – 60 degrees
- Curves reduce to ≤ 30 degrees on lateral side-bending x-rays
- Kyphosis < 55 degrees (measured from T5 to T12)

Time is key when it comes to scoliosis surgery. ApiFix is most effective when performed during a child’s growth period, which is typically between the ages of 10 and 16. Your provider will consider your child’s age, growth plates, curve severity and flexibility to find the ideal window for surgical intervention.
Like any surgery, ApiFix comes with some risks, but for the right patient, it can be a safe and effective option with fewer long-term trade-offs than traditional spinal fusion.
Potential risks of ApiFix include:
- Infection at the surgical site (rare and typically treatable with antibiotics)
- Implant-related complications, such as device movement or breakage
- Curve progression if the spine doesn’t respond as expected
- Reoperation in some cases, either to adjust or remove the device
Other important considerations:
- Not all patients are candidates: ApiFix works best for children or teens with flexible, moderate curves (35°–60°).
- Long-term data is still growing: While early outcomes are promising, ApiFix is a newer alternative to fusion, so families should consider both the current benefits and the evolving research.
- Motion preservation vs. stability: ApiFix allows more spinal motion than fusion, which many families prefer, but it may not be suitable for severe or rigid curves.
