• If your child is having difficulty falling asleep, please call 937-641-5004, and select option 2.
• 12 months old or greater with concerns of snoring, pauses in breathing, daytime sleepiness, gasping or choking during sleep, or other sleep concerns please use the online scheduling tool below. 
• Under 12 months of age please call 937-641-5004, and select option 2.
