Objectives:
- Define health equity and the social drivers of health.
- Identify how differences in social drivers of health contribute to persistent chronic disease disparities among racial, ethnic, and socioeconomic groups, systematically limiting opportunities for members of some groups to be healthy.
- Discuss approaches to screening for social needs.
- Access resources to address social needs.
