CENTER FOR MEDICARE AND MEDICAID SERVICES (CMS) OUTLINES CONDITIONS OF PARTICIPATION (COP) UPDATE:
The Center for Medicare and Medicaid Services
outlines a number of Conditions of Participation for hospital-based staff
who care for Medicare patients. As a participating hospital, we are
required to comply with all of these COPs. One such COP states that all
patient medical record entries must be legible, complete, dated, timed and
authenticated in written or electronic form by the person responsible for
providing or evaluating the service provided, consistent with hospital
policies and procedures. As an institution we do well with each of these
components with the exception of timing the entry. A recent chart review
identified 28 entries by a number of disciplines. Only 14 of the 28 (50
percent) entries included a time with the entry. We are taking additional
steps to assure that all entries are timed. We appreciate your help in
complying with this COP. Questions? Call Tom Murphy, MD, at
937-641-5871.
